One way the NHS can improve outcomes is from recommendations in coroners reports on deaths while patients were in the care of the NHS.

However, in the case of maternal deaths it appears that most of the recommendations made over the ten years between 2013-23 have not actually been acted on at the national level.

Repeated warnings on training & escalation protocols have been ignored;

yet one more instance when our health service fails women & children
