Here's the coincidence of two major problems - the costs of US health care, and the difficulty of getting (travel) insurance firms to pay up claims (especially in medical related areas).

Having had a child delivered early in the US (and needing time in intensive care) costing around $200k, the insurance company spent some time denying the parents claim before finally paying up.
